How to Fax Blue Cross Blue Shield

Before anything else, the thing that trips up almost everyone:

There is no single Blue Cross Blue Shield fax number. BCBS is a federation of independent, locally operated companies -- Blue Cross Blue Shield of Illinois, of Texas, of Michigan, Anthem plans, Highmark, Premera, and more. Each one runs its own departments and its own fax lines. A number you found for one state plan will not reach another.

So the real task is finding *your* plan's *department's* number. Here is how.

Finding your BCBS fax number

1. Your member card. Look at which BCBS entity issued it -- "BCBS of Texas," "Anthem Blue Cross," "Highmark," and so on. That is the company you are dealing with, and it determines everything else. Call the member services number on the back and ask for the fax number for your specific need.

2. The document that told you to fax. A denial letter, an Explanation of Benefits, a prior authorization form, or an appeal notice almost always prints the correct destination fax number. That number beats every other source, because it is already routed to the right department.

3. Say what you are sending when you call. BCBS plans typically run separate fax lines for:

  • Claims submission
  • Appeals and grievances
  • Prior authorization (often split again into pharmacy vs medical)
  • Provider or practitioner services
  • Member services and general correspondence

"I need a fax number" gets you the wrong one. "I need the fax number for a pharmacy prior authorization" gets you the right one.

Do not reuse a number from an old letter without confirming it, and do not use a general claims fax for an appeal. Misrouted paperwork tends to vanish quietly, and appeal deadlines do not pause while it sits somewhere.

What to include

  • A cover sheet with your name, member ID exactly as printed on your card, the claim or reference number from your letter, date of birth, and a callback number. Our free insurance cover sheet has the fields laid out.
  • The reference number for whatever you are responding to. This is what lets them file your fax against the right decision instead of starting a new one.
  • Every page, signed where required, right side up and legible.
